Dear Students!
Please be informed that Exam on Orthopedics and Traumatology will be held on Friday, May 14th 2010 at 13:30 at the Seminar room of the Department of Orthopedics and Traumatology.
Faculty of Medicine with Division of Dentistry
and the Division of Medical Education in English
In order to be issued a diploma, all students in their last year of studies must provide the Dean’s Office with the following documents:
- Index (Indeks) – must be completed with all required signatures;
- 6th Year Examination Card Sheet – must be completed with all required signatures;
- Clearance Form (Karta Obiegowa) – filled out with all required stamps and signatures. This card is valid for two weeks, therefore it is highly recommended to complete it at the end;
- 5 Photos (4,5 cm x 6,5 cm) for copies of the diploma in Polish and in English – formal dress is required, and the picture should be taken on a white background;
- Official written request for issuance of the student’s Diploma and Diploma supplement in English Language; a sample copy of this letter is attached to this document. Please see the next page;
- Payment in the amount of 100 PLN for the diploma fee – which should be paid at the cash desk in the Palace;
- Student’s ID.
All documents must be provided in a complete set at the same time. The diploma must be received in person by the graduate himself/herself. Diplomas will be issued within 30 days after all required documents have been received.
